Description: This article guides a voluntarily terminated Team Member through completing the exit form.

1. If a Team Member is voluntarily leaving the company, they will receive an email with the company logo with a link to complete the Exit Form.
2. The Team Member will complete the form at their own discretion, and then select "Next" to continue.
3. The final form will thank you for completing the form. Select "Submit" to send the form and complete the process.
