When Does This Solution Apply: A Team Member is enrolling into their benefits.
Description: This article will guide any user who is eligible for benefits and has not yet selected their benefits for the upcoming year through the process of open enrollment for Voluntary Life and AD&D. Keep in mind, this is Voluntary life and AD&D in reference to you, not your spouse or children. 1. You have the option of additional Employee Paid Voluntary Life and AD&D Insurance. Since the beneficiaries of your Voluntary Life and AD&D must be the same as your Basic Life, you must complete the Basic Life section before you see the Supplemental Life section.
2. If you are currently enrolled in Voluntary Life and AD&D, your current elections will display in both the life Insurance Requested and Life Insurance Approved fields.
a. Enter the total amount of Life Insurance you wish to purchase. You can select up to $100,000 without an Evidence of Insurability determination. If you enter more than $100,000, any amount above the guarantee issue will show in the Life Insurance Additional field.
NOTE: An error will occur if "Life Insurance Requested" field is not in "10000" increments.
3. Beneficiary information must be the same as your Basic Life Insurance, so there is no entry for beneficiaries in Voluntary Life and AD&D.
b. Click Recalculate and Cost. The cost will calculate for the Approved amount only.
