  • Once opened, you will view the below page.
  • From here you will be able to navigate to any of the forms mentioned above.
  • The options on the left help you navigate to the forms submitted (My inspections) and forms in progress.
  • The home page of the website gives you an overview of all the forms you have completed, forms that you have in progress, the average score of submitted forms and the number of inspections that need improvement.



  • When completing a Safety Assessment, it is important to note the total passing score is 68%
  • Each question on the Safety Assessment form provides three answer options-


1. #KeeleySafe - This answer is worth a score of "1"


2. Improvement Opportunity - This answer is worth a score of "0"


3. N/A - This answer is worth a score of "0"


How the final score calculation works:


The final score on your assessment is calculated based on the number of answers you marked #keeleySafe divided by the maximum possible score for the categories you have chosen multiplied by 100.


Final Score Percent = ( Actual Score / Maximum Score ) * 100


Example- If you perform an assessment for the category Jobsite Access/WZTC, the maximum score you can get is 5 because there are 5 questions in that category.

- if you scored 4 / 5, meaning you marked 4 questions as #keeleySafe----> your score would calculate to -

(4/5) * 100 = 80%



Colors on the PDF or Submitted forms


  • After you have submitted the assessment and download the PDF or simply viewed the submitted form, you might notice the green and red colors on the answers you provided. 
  • The color green denotes being #KeeleySafe
  • The color red denotes the areas of improvement
