Safety One Stop / Safety App (Document for Team Members)

Purpose: The purpose of this document is to guide you with instructions to access the Safety Assessment Form.

About Safety One Stop: This website is created for keeleyn’s to easily access forms like – Good Catch, Safety Assessment, COVID- 19 Awareness Assessment and Coaching Session Form.

Who is this for: All team members who want to submit a Safety Assessment.

Accessing Safety one Stop website:

Open your browser and copy this link - https://safetyonestop.keeleycompanies.com

• Once opened, you will view the below page.
• From here you will be able to navigate to any of the forms mentioned above and in this case the Safety Assessment Form.
• The options on the left help you navigate to the forms submitted (My inspections) and forms in progress.
• The home page of the website gives you an overview of all the forms you have completed, forms that you have in progress, the average score of submitted forms and the number of inspections that need improvement.

• To access a form, click on forms on the left section. This will open all the forms that are available. 
• In this case, navigate to the Safety Assessment Form and click on the arrow.

• Clicking the arrow will begin the Safety Assessment form
• You should see the below screen-

• This is the first page of the Safety Assessment form. 
• You name will get auto filled on the form.
• If you need to select another company, simply click on the drop-down arrow on the right

• That should show you the below options. Scroll up or down to see the entire list.

• Depending on which company you select, the options on the screen will change.
• You will then need to select the appropriate group for the company by clicking on the arrow on the right and scrolling up or down to see the entire list.
• Depending on which group you select, the options on the screen will change.

Based on which options you select, the questions that follow will change-

• Example- if you select ADB Companies, group OSP East, you may need to enter the Job Number and then select the correct VP.  The Client gets auto filled based on the job number you enter.
• Example- if you select Keeley Companies, group KeeleyCares, you will be required to enter the Project Name/ Location but not the Job Number. 
• Example- if you select L Keeley Construction, you will be required to enter the Job Number to proceed further.

• If you check the box for Subcontracted Work, you will be asked to enter the Subcontractor Name.

• You will not be asked to fill in the Subcontractor Name if you do not select the checkbox for Subcontracted Work.

• You will then need to add the name of the Supervisor or Superintendent to move further in the form. You can choose the supervisor or superintendent by clicking on the drop down arrow on the right side.
• You could also start typing the name of the individual in the text box to filter the names.

• The conducted on date and time get auto filled based on when you are filling the form.

To save the form and submit later, click on the green button to save the form and close out of the website.

To access the saved form again, click on the "In Progress" tab on the left side of the screen.

Look for the form you were trying to complete by looking at "Conducted On" 

Once you have identified the form, click on the pencil icon to start completing the form again

That will open the Safety Assessment form where you left off. 

To move to the next page, click the arrow-

• The second page of the safety assessment form will ask you to select a category for which you wish to perform the assessment. You will not be able to move forward without selecting at least one category. 
• You can either select any one by checking the box next to the category name or you can select multiple categories.
• Based on which category you select or de-select, the options at the bottom of the page will change.

Example- if you want to submit an assessment for the categories "Flammable Liquids" and "Environmental" you will select both the check boxes and then answer the questions below each category you selected.

• Each question on the Safety Assessment form provides three answer options, each having a score of either 1 or 0.

   
  1. #KeeleySafe - This answer is worth a score of "1"
  
  2. Improvement Opportunity - This answer is worth a score of "0"
  
  3. N/A - This answer is worth a score of "0"

• For each of the questions, you can attach notes or images by clicking on either of these two icons. 
• Clicking on the yellow icon, lets you add notes
• Clicking on the blue icon, lets you add images / files

• When adding a note, click on the yellow icon that is circled above. It should open the below pop-up window.
• Type what you would like and then click on the icon that is circled.
• Your note will not be saved, if you do not select that icon. 

• Once your note is saved, you should see the below pop-up window that should confirm your note is saved.
• You should also see the note added on the right side of the screen.
• You could edit the note by clicking the pencil icon or delete it by clicking the red icon.

• Adding notes, images or files will change the number from 0 to 1 on the the right side of the screen.

Note- We are aware that some of you have been facing issues with attaching multiple images or files where the storage capacity gets maxed out or the pictures just do not get attached the the pdf. 

We are currently working on a solution where you will no longer experience this issue. Thank you for being patient!
