Safety One Stop / Safety App (Document for Safety Admin)
Purpose: The purpose of this document is to guide you with instructions on how you could add questions to a previously saved template on the Safety App.
About Safety One Stop: This website is created for keeleyn’s to easily access forms like – Good Catch, Safety Assessment, COVID- 19 Awareness Assessment and Coaching Session Form.
Who is this for: The Safety Admin
Accessing Safety one Stop website:
Open your browser and copy this link - https://safetyonestop.keeleycompanies.com
- Once opened, you will view the below page
- From here you will be able to navigate to any of the forms mentioned above, view all completed and in progress forms
- You will also have the ability to add questions to a previously saved templates
- The options on the left help you navigate to the Safety Admin section where you can create new forms, edit existing forms and templates.
- Once you have found your template from "Draft templates", click on the pencil icon to edit it / to add questions
- Once you hit edit, you will see the below screen that asks you to enter the Section Title
- Enter the title
- To add the questions below the title, click the second icon
- Once you click that, you will be presented with options on the type of question you want to add
- The type of question can be different for every question that you add
- If you want to add a Short Text question type, select short text
- That will open the below option for you
- Here you can type your question
- After you have typed your question, you have 3 question options. Click on the arrow to view the options.
1. Answer Required- The user will not be able to proceed if the answer to this question is blank
2. Allow Note- This option will allow the user to add notes to the answer
3. Allow Attachments- This option will allow the user to add attachments like images or files.
- You can select the option by checking the box
- To add another question, click on the icon again
- Choose the type of question you would like to add
- Selecting Single Select, will prompt you to type in the question
- Since this is a single select type of question, you are required to provide the answer options for the question
- To add the answer options, click on the drop down next to "Question Answers"
- Then click the icon to add the answer options
- Type in the answer options
- Keep clicking the icon to add answer options
- You can also add colors to each of the answer options. Example- if the answer is yes, it will show as green in the pdf
- Here as well, you have been given the below options to choose from-
1. Answer Required- The user will not be able to proceed if the answer to this question is blank
2. Allow Note- This option will allow the user to add notes to the answer
3. Allow Attachments- This option will allow the user to add attachments like images or files.
- You can select the option by checking the box
- You also have another option "Question Logic"- meaning you have the option of showing that particular question if it meets some condition.
- Example show this question if the answer to the first question equals yes
Click on the drop down next to Question Logic and then click the icon to add the logic
- You can delete the logic by hitting the 'X' next to the logic
- To add another question, click on the second icon again and select the type of question
- If you select the question type "Date Picker", it lets the user select a date
- Type your question, select the options if you want the user to add notes or images
- Add the logic if necessary
- In the same way you could add a question to ask the user to enter a time stamp
- Select the question type- "Time Picker"
- In the same way you could add a question to ask the user to pick an answer from multiple options
- Select the question type- "Picklist"
You also have an option to rearrange the answer options

To do so, ensure the lock icon on the top right corner for the question is unlocked
2. You then hover on the title of the answer (Answer 1, Answer 2, Answer 3....) until you see the four faced icon
3. Once you see that, click, hold and drag the answer option to where you would like to see it
- If you would like to add another page to the form, click on the right facing arrow at the top
- That will prompt you to add another section title
- Here you can follow the same process of adding the questions
- If you select the question type "Question group", you can group questions that have the same answer options
- Example- you could add 3 questions :
1. Do you wear face masks?
2. Do you like wearing face masks?
3. Do you go to the mall without masks?
- All the questions can have the same answer options- Yes, No, May be
- Once saved the actual form would look like below to the user
To delete a question, simply click on the 'X' next to the lock icon for that question
To rearrange the order of the questions:
- Click on the icon that is circled in the image below
- You should then see the options available to you
- Click on "Order Questions"
- You will then see a pop-up window with all the questions on your form
- Hover on the question you want to rearrange
- Once you see the four faced arrow, drag and move the question to where you would like to see it
- Example- Here we switched the order of question 5 and 6 (when compared to the image above)
- Click Ok
- Verify if the order is as required
- Click the save button
To save the template/form, click on the green icon at the top
