Safety One Stop / Safety App (Document for Safety Admin)
Purpose: The purpose of this document is to guide you with instructions on how you could add scoring to the answer options on a previously saved template/form on the Safety App.
About Safety One Stop: This website is created for keeleyn’s to easily access forms like – Good Catch, Safety Assessment, COVID- 19 Awareness Assessment and Coaching Session Form.

Accessing Safety one Stop website:
Open your browser and copy this link - https://safetyonestop.keeleycompanies.com
- Once opened, you will view the below page
- From here you will be able to navigate to any of the forms mentioned above, view all completed and in progress forms
- You will also have the ability to add scoring to the answer options on a previously saved templates
- The options on the left help you navigate to the Safety Admin section where you can create new forms, edit existing forms and templates.
- Once you have found your template from "Draft templates", click on the pencil icon to edit it so you can add the scoring.
- Once you hit edit, the form would open to the questions page
- Click on the icon that is circled in the below image
- You should see the below options
- Click on Scoring
- You should see the below pop-up window
- You are required to check the box to "Allow Scoring" before you can enter the passing score
- Once checked, enter the passing score for that form. Example- 50. Anyone who gets a score of less than 50, would need improvement.
- Once entered, click "OK"
Note- You can add scoring to questions that are single select and multi-select
- Look for the questions that are either single or multi select types
- Once you find them, click on the option "Questions Options"
- You should see a new available option- "Allow Scoring"
- Check the box
- Once checked, click on the arrow to collapse the view
- Click on "Question Answers" to open the answer options
- Here you can assign the score for each of the answer options
- You could also check the box to "Ignore Scoring" for any of the answer options. Example- If you do not want the user to lose points if they select the answer "NA" for a question, you could check the box to ignore scoring for that option.
- Once you have completed adding the scoring, ensure you click the "Save" icon at the top of the page
