Purpose: The purpose of this document is to give Team Members an overview of the LKC New Hire Portal Arizona A4 screen and guide you with instructions to walk through this process.

About LKC New Hire Portal: This website is created for Keeley new hires and Managers to easily walk through and complete the onboarding forms within the LKC New Hire Portal. Once completed, the information will be sent into our HR System, Sage People and then will transfer into our Accounting System, Sage 300 CRE.

PLEASE NOTE: Any questions pertaining to how to complete this form should be directed to the Payroll and HR Teams.

 

When you select Arizona on the previous W-4 State Selection Screen you are brought to the screen below to complete the Arizona State A4 form.

  • Instructions for the form are on the top and bottom of the screen
  • It is required that you make a selection for withholding status and complete the signature field before clicking Next.
  • When you choose “Withhold from gross taxable wages at the percentage checked” you can select any of the options listed by selecting the button next to the percentage you would like.
  • You can also choose the “Other” option and enter any amount you would like to withhold.
