Safety One Stop / Safety App (Document for Team Members)
Purpose: The purpose of this document is to guide you with instructions to access the Event Report on the Safety App.
About Safety One Stop: This website is created for keeleyn’s to easily access forms like – Good Catch, Safety Assessment, COVID- 19 Awareness Assessment , Coaching Session and Event Report.
Who is this for: All team members who want to submit a Event Report to the Safety Team
Accessing Safety one Stop website:
Open your browser and copy this link - https://safetyonestop.keeleycompanies.com
- Once opened, you will view the below page.
- From here you will be able to navigate to any of the forms mentioned above and in this case the Event Report.
- The options on the left help you navigate to the forms submitted (My completed), My in progress, My In Review and My Finalized.
- The home page of the website gives you an overview of all the forms you have completed, forms that you have in progress, the average score of submitted forms and the number of inspections that need improvement.
Click on the menu item Forms to submit an Event Report.
Note- the event report works slightly different than the other forms because the Event Report needs to be submitted for review to the safety team.
Once you click on forms, you should see the below screen that shows all the forms available to complete and submit
Scroll up or down to look for the Event Report form
To begin the form, click on the button at the bottom
Once you begin the form, you will be presented with the General Information Page
- Your Name, Company, Date and Time will be auto-filled on the form. But you have the ability to change that.
You can change the company by clicking on the drop down menu on the right side
- Doing so will open the list of companies to choose from
After selecting the company name, you will be required to select the Group for that company.
-Based on the company you select, the group names will change
After you have the information filled in, you can either the save the form and return to it later or you can click on the arrow at the bottom of the page.
- If you want to save the form, click on the green button at the bottom. You can then find your form in the "My In Progress" section
- If you want to continue completing the form, click on the arrow button. That will take you to the next page of the form
When you move to the next page you will be asked some specific information about the Event Type you are reporting.
Note- Based on what type of event you select, the questions on the form will change. The total number of pages on the form will change as well.
Example- When you are on the 2nd page of the form, you see there are a total of 6 pages.
As soon as you select the Event Type, the number of pages will increase based on the type of event you are wanting to report
Example- If Motor Vehicle Incident is selected, the number of pages go from 6 to 8. You can also select multiple options here, like Motor vehicle and Injury
Based on the answer selections you make, the questions further down in the form will change
Example - when there is no Event Type selected, we see the second question is "Date of Incident". As soon as we select Motor Vehicle Incident, another question shows at the bottom
Once you have filled out the details, you can move to the next page by clicking on the arrow at the bottom
When you move to the next page, you will be required to fill out all the details as needed
For questions like these, make sure to hit the drop down menu to select from the available options
After you have filled in your report and reach the last page of the report, you will see a screen like the below image-
This screen has a button asking you to "Submit For Review". On clicking this button, you would have submitted the form to the Safety Team for review.
Note- The form gets automatically sent to the safety team, you do not need to do anything further to submit this form unless specifically asked by the safety team
Once submitted, you will see that this form will be available for you in "My In Review" section until the Safety Team approves it.
Once the safety team approves it, you will no longer see this form in "My In Review".
You will see it in "My Finalized"
