Purpose: The purpose of this document is to guide Team Members with instructions to access and submit their Covid-19 Vaccination Form in SagePeople


About Covid-19 Vaccination Form: This form allows you to enter basic information on the vaccine you received, when you received your doses and also allows you to attach your vaccination card.

Accessing the Form: 


Navigate to your SagePeople account-

  • Once there, you should see the below page



  • Look for the menu item "Forms". If you do not see it, click on the dropdown for "More"



  • Click on Forms


  • Click on View Details



  • Click "New"



  • You will then see a pop-up window like the one below
  • Click on the drop down arrow next to Available Forms



  • You will see either Keeley Companies COVID-19 Vaccination Form or ADB Companies COVID-19 Vaccination Form.
  • In this case it is Keeley Companies COVID-19 Vaccination Form
  • Select it and click Start to begin the form




  • You will then see a screen like the one below for a Keeley Companies covid-19 form



  • If you decided to not get vaccinated, click on the check box at the very top 
  • Click next


  • Click Submit 

Once submitted, close the window.



  • If you got vaccinated, select the vaccine manufacturer from the drop down list
  • Fill in the dates for the doses you received
  • Attach your Vaccination Card by clicking "Choose File". Verify the card is attached.
  • Click Next



  • Click Submit 
  • And Close the browser




Once submitted, close the window.
