Purpose: The purpose of this document is to guide Team Members with instructions on how they can view the information they submitted on their Covid-19 Vaccination Form in SagePeople
About Covid-19 Vaccination Form: This form allows you to enter basic information on the vaccine you received, when you received your doses and also allows you to attach your vaccination card.

Accessing the Form:
Navigate to your SagePeople account-
- Once there, you should see the below page
- Click on "My Details"
- Click on "View Details"
- Then click on Edit
- That should open a pop up window like the one below
- Scroll to the bottom to view the details you submitted on your form
NOTE---> You will not be allowed to edit these details once submitted. You will need to reach out to your HR team if you need to make changes.
After the HR team has made the change you can re-submit the form by navigating to the "Forms" menu item.
