Safety One Stop / Safety App 

Purpose: The purpose of this document is to guide you with instructions on how you can create custom alerts that pop up when a particular answer option is selected.

About Safety One Stop: This website is created for keeleyn’s to easily access forms like – Good Catch, Safety Assessment, COVID- 19 Awareness Assessment, Coaching Session Form and Event Report.

Who is this for: Team Members with Admin role 

Accessing Safety one Stop website:

Open your browser and copy this link - https://safetyonestop.keeleycompanies.com

  • Once opened, you will view the below page
  • From here you will be able to navigate to any of the forms mentioned above, view all completed and in progress forms 
  • You will also have the ability to access the admin/ back-end of SOS
  • The options on the left help you navigate to the Admin section where you can create new forms, edit existing forms and create custom alerts 




  • To add an alert to an answer option, open an existing form you want to add the alert to or create a new form
  • To add the alert to an existing form that is in draft status:
  • Click on the drop down arrow next to 'Admin' to view the available options
  • Navigate to 'Draft Templates'



  • Open the form you want to edit by clicking on the pencil icon 


  • Clicking on the pencil icon will open the page with questions on it
  • If there are no questions, add a question (single or multi select or picklist) to attach a custom alert to it


NOTE: Custom Alerts can only work for Single select, Multi select and Picklist question types


  • When you add either of the 3 question types, you will see a new option 'Custom Alerts' that is available below the answer options


  • Add the answer options to the question you added


  • Once added, click on 'Custom Alerts'
  • That will open the below screen
  • Click on the '+' icon to create a new alert for that question





  • Doing so will display the below 



  • From here, select the answer option that should trigger the alert
  • Click on the drop down arrow to see the answer options you added to the question


  • Once selected, navigate to the next item that reads 'Alert Message'
  • Click on the hint text to open the editor so you can type the alert message
  • Click on the 'View' icon next to the hint text to open the editor so you can type the alert message


  • Clicking on the hint text will open the below text area
  • Type in the alert message you want the team member to see
  • You can use any of the functions like 'bold, italics, underline' 



  • To collapse this text area, click on the 'Alert Message' hint text again


  • Navigate to the 'Alert Action' drop down


From here, you have two options:


  • You can either stop the Team Member from moving forward if they select 'No' as the answer option for a particular question or
  • You can allow the Team Member to complete the form even if they select 'No' as the answer option


  • After you have made the selection, you have the option of selecting an alert icon that will show above the alert message



  • After you have made the selection, you can preview how the alert would be displayed to the Team Member
  • Click on the 'View' icon 




  • On click, it shows the below pop up


In this example, when the team member will select 'Yes' as the answer option, they will see a pop up like this one


  • You can add another alert to the second answer option as well


Once completed hit the save button


  • To view what this would look like to the team member, navigate to draft templates after saving your changes
  • Click on the magnifying glass to view the form 



  • Enter test information and navigate to the page with the questions
  • Select 'Yes' as the answer and you should see the pop up with the message you added
  • Click 'Ok'


  • If you chose 'Stop from moving forward' while creating the alert, as soon as you click 'Ok', the selected answer option will uncheck automatically and the user will not be allowed to move forward unless they select another answer option
  • After you have verified the alert works as you expected, publish the form
